By signing below, the signatories testify on behalf of themselves and the training center:

  • All the information provided in the application form and all The required documentation and materials are correct.

  • I have read the MSDI minimum standards, and the training center meets the requirements.

  • I understand that if I do not meet the MSDI application requirements, the MSDI application will be automatically denied.

  • I understand that such a determination will be based on MSDI's sole discretion. I understand that if the application is rejected, I can only submit another application after 24 months
