Quarterly Training Evaluation - Guide Dog Tasks

The dog's performance must be rated from 1 to 10
(1 represents the worst and 10 represents the best)

*Filling out this test form will allow you to segment the data, identify the dog's strengths and weaknesses and build the dog's progress graph based on this data.

COMMAND

1-10

1. Lead handler along familiar route

2. Take handler to specific entrances

3. Alert handler to low-hanging obstacles

4. Alert handler to elevation changes

5. Alert handler to street or Intersection

6. Assist handler in finding known Items

7. Notify handler of items in path

8. Guide handler around hazards (open manhole cover)

9. Avoiding moving objects (bikes, skateboards, etc)

10. Indicate stairs, steps, or ramps
